My vintage loving self does *not* want you to change it.
But, you could take two of the tapered baguettes and use them as side stones.
You could have the rest set into earrings.
Main stone in the middle and the smaller ones as baguettes on either side
Oohhhh… I’d do the large stone as just a very simple solitaire on a plain band and then make the others into your wedding band, either deliberately uneven and just recessed into a band lengthwise, or a sunburst halo like this:

That’s so cool!! I love the baguettes and selfishly want you to leave them exactly where they are. But you COULD remove the round diamond and reset it however you’d like. I think the ring would still be cool without the round diamond.
OMGosh I would not change a single thing about that ring, it’s so unique I love it!
I would take the main stone out, then I would try to rework the original ring to basically be kept in it’s original style (just without the setting for the large stone). Its a pretty interesting style and if you’re not big on jewelery you may appreciate having the original ring with its sentimental value instead of just other new piece of jewelry you may not wear often.

I’d personally remake it. There’s a lot of gold to work with. I’d ask the jeweler to melt it all down and make the engagement ring AND wedding band with it. I’d personally use the small round diamonds and Gypsy-set them into the wedding band. I’d use two of the baguettes for side stones with E-ring
I would leave it. If you must I would put the round diamond in a simple solataire and keep the rest of the ring as is after removing the prongs.
You could still wear them together sometimes but also have a more simple option
To add onto the idea of taking out the main diamond to use in a new ring/piece of jewelry, you could replace it with an inexpensive stone of your choice

Thought about altering it for me at first, but I decided not to in the end. It’s been 4 years and I am so glad I didn’t.
Everytime I see it and wear it it reminds me of her and seeing it on her when she was alive.

People on here always balk at the idea of changing vintage rings and I’m in total agreement if it’s a genuine antique from a century ago, but these rings from (I’m guessing) the 70s-80s are so bulky and that’s not everyone’s style😭 I would do a solitaire with the main diamond and maybe you could also use two of the tapered baguette diamonds like this?

I think the older rings like this are SO COOL. They’re unique and bold in a way you don’t see anymore.
If you’re really set on rebuilding it I would go on Pinterest and just type in a few words to describe what you generally like, and keep refining from there.

The tiny diamonds are pretty useless, but the tapered baguettes would make nice side stones in a new setting. I don’t care for the ring, but it is certainly unique. Do you know when it was given to your grandmother?

You could take the main diamond and turn it into a simple solitaire ring or pendant necklace you could wear everyday and leave the rest of the ring as is to be worn as a fun cocktail ring kind of like this!

I would considering killing someone for that ring as-is. I’m not saying I *would,* but is definitely think about it.
If she likes vintage stuff at all you should leave that the way it is. I would propose with it like that and design something new together if she’s not into it. You could probably remove the head holding the round stone and just have the band as its own ring.

I had a similar situation inheriting diamonds and made this 5 stone ring. You could do something similar!
This is striking! If it were me I’d keep it complete. However, I think using the large stone for a new ring and replacing it with some sort of colored gem in this one could be really cool. You get the ring you want and you’re keeping the integrity of the antique.
