It’s typically the second, but there isn’t a rule that says you can’t wear it the other way. I’m a fan of do what you want.

I wear mine like the 2nd photo.
As a matter of fact, anyone I’ve seen with a pear shaped wears it pointy side pointing away from hand
If you go look on jewelry sites, the point is always pointing towards your nails, as in the second picture. The first would usually be considered upside down. That being said, wear yours however the heck you feel like it. Switch it up from day to day if you can’t decide or like it both ways.

There’s no standard.
Some people like to have the point going “out” because it visually elongates the finger.
I wear mine with the point going “in” because I want the ring to point towards my heart.
Both ways are beautiful, but the second pic is my vote!
